Who We Are
Care Path CFSS Consulting LLC (“CarePath,” “we,” “us,” or “our”) is a Minnesota-based CFSS consultation company located at 2649 Park Ave South, Suite 204, Minneapolis, MN 55407. We provide Community First Services and Supports (CFSS) consultation services to individuals and families across Minnesota.
As a CFSS consultant working with participants enrolled in Minnesota Medical Assistance, CarePath is considered a Business Associate and in some contexts a Covered Entity under the Health Insurance Portability and Accountability Act (HIPAA). This means we are legally required to protect the privacy and security of your Protected Health Information (PHI).
Information We Collect
We collect only the information necessary to provide CFSS consultation services and to comply with Minnesota DHS program requirements.
Personal Information
- Full name, date of birth, and contact information (address, phone, email)
- Minnesota Medical Assistance (Medicaid) ID number
- Emergency contact information
- Preferred language and communication preferences
- Authorized Representative information (if applicable)
How We Use Your Information
- CFSS Consultation Services: To develop and maintain your service delivery plan, provide consultation, and support your CFSS goals
- Program Compliance: To meet Minnesota DHS documentation, reporting, and audit requirements
- Communication: To contact you regarding appointments, plan updates, annual orientations, and service changes
- Referral Coordination: To coordinate with your case manager, MCO, or county with your consent
- Quality Improvement: To review and improve our services internally (using de-identified data where possible)
- Legal Compliance: To comply with applicable federal and Minnesota state law

- Secure document handling and shredding procedures for paper records
- Business Associate Agreements with all contracted partners who handle PHI
- Breach notification procedures — we will notify you within 60 days if your PHI is involved in a breach
