Every person receiving CFSS services has protected rights under Minnesota law. CarePath is committed to making sure you know them, understand them, and can exercise them at any time.
The rights listed on this page are grounded in Minnesota Statutes and DHS CFSS program requirements. If you believe your rights have been violated, CarePath can help you understand your options. You may also file a complaint directly with the Minnesota Department of Human Services. Contact CarePath if you have any questions.
Simple, fast, and built around your client's needs.
Every person receiving CFSS services has protected rights under Minnesota law. CarePath is committed to making sure you know them, understand them, and can exercise them at any time.
You have the right to choose your CFSS service delivery model — Agency Model or Budget Model — and to change your choice if your needs or circumstances change.
You must be given clear, complete information about your CFSS options, rights, and responsibilities before making any decisions — in a language and format you can understand.
You have the right to be treated with dignity, respect, and courtesy by all CFSS consultants, support workers, and program staff at all times — without exception.
If you are unhappy with your services, your consultant, or any part of your CFSS experience, you have the right to file a complaint — without fear of retaliation or loss of services.
If your CFSS services are reduced, denied, or terminated, you have the right to appeal that decision through the Minnesota Department of Human Services fair hearing process.
You have the right to receive CFSS information and services in your preferred language, including the right to request a qualified interpreter at no cost to you.
You have the right to be free from physical, emotional, sexual, and financial abuse, neglect, and exploitation by any support worker, consultant, or program staff.
CFSS is designed to support your full participation in community life. You have the right to services that promote independence, community access, and the life of your choosing.
Person-centered planning means that every decision about your CFSS services starts with you. Your goals, your preferences, your daily routines, and the life you want to lead are the foundation of your service delivery plan — not what is easiest for a provider or required by a checklist.
At CarePath, protecting your rights is not just a compliance requirement — it is the foundation of how we work. Every CarePath consultant is trained in DHS participant rights requirements and committed to the following standards:
Our team is happy to speak with you directly about how CarePath works and whether we're a good fit for your client's needs.
